Chi Life Wellness Association — Membership Agreement

1. The Association

Chi Life Wellness Association (the "Association") is a private membership association formed under its Articles of Association, which are incorporated into this Agreement by reference and available from the Association on request. The Association offers wellness education, coaching, and related programs to its members through Chi Life Wellness ("the practice").

2. Becoming a member

Membership is by application and approval. You apply by completing registration, accepting this Agreement, and paying the program fee. Membership begins when the practice approves your application and continues for the duration of your program, and afterward while your account remains active. The Association may decline an application or end a membership as set out in Section 10.

3. Private program

The Association's programs and materials are offered privately, to members only, and are not offered to the general public. As a member you receive access to the 9-week program, its materials, member pricing, and the member portal. Membership in the Association does not change the legal protections that apply to your health information: the practice maintains the privacy and security safeguards described in its Privacy Policy, and nothing in this Agreement waives rights you hold under applicable health-privacy and consumer-protection law.

4. Fees

The membership program fee is $895 for the 9-week program, due at enrollment unless the practice agrees otherwise in writing. Fees for additional programs, sessions, or products are shown before you buy. Refunds are handled as described at the point of purchase or, when not stated, at the practice's published refund policy.

5. Member responsibilities

Provide accurate information during enrollment and keep it current; use program materials for your own personal, non-commercial benefit; do not share your account or redistribute member materials; treat staff and other members respectfully in shared spaces.

6. Health acknowledgments

The Association's programs are wellness and lifestyle education, not medical care, and program materials are general guidance rather than medical advice for your individual situation. Consult your own physician before starting any weight-loss program, and complete any health screening the practice asks for truthfully. Tell the practice promptly if your health circumstances change during the program.

7. Privacy and records

The information you share and the records created during your program are handled as described in the practice's Privacy Policy, under the safeguards that apply to health information. The Association does not sell member information and does not use it for third-party advertising.

8. Resolving disputes

We ask that concerns be raised with the practice first — most issues are resolved by talking. If a dispute cannot be resolved informally within thirty days, the parties agree to attempt mediation before filing any claim. This section does not limit any right you hold under law that cannot be waived by agreement, and does not prevent either party from seeking emergency relief a court can provide.

9. Liability

Individual results vary and are not guaranteed. To the maximum extent permitted by law, the Association's and the practice's liability arising out of this Agreement is limited to the amounts you paid in the twelve months before the claim. Nothing in this Agreement excludes or limits liability that cannot be excluded or limited under applicable law.

10. Ending membership

You may withdraw from the Association at any time by written notice to the practice; withdrawal does not by itself create a right to a refund beyond the refund policy in Section 4. The Association may end a membership for material breach of this Agreement or conduct harmful to members or staff. Ending membership does not erase the record-keeping obligations the practice carries for services already delivered.

11. Changes to this Agreement

The Association may amend this Agreement. Material changes will be published on this page with a new version date, and you will be asked to review and accept the updated Agreement before continuing to use member services.

12. Acceptance

You accept this Agreement by checking the acceptance box presented at registration or checkout. That electronic acceptance is your signature, and the practice keeps a record of the version you accepted and when.
